Please fill out this form as completely and accurately as possible so we can get to know you and your cat(s) before your visit.

This form is intended to confirm your plans to board your cat(s).
You are not required to bring anything with you except the following:
  • Your Cat(s)
  • Carrier(s)
  • Medication(s)
  • Special Diet(s)

We have our in-house food, as well as bedding, dishes, and toys here. You are welcome to bring any special items your cat(s) may prefer. Please label these items so that they are returned during pick-up at the end of the boarding period.
Please let us know if you would like any procedures done during the boarding stay. This may include:–
  • Nail Trims
  • De-Matting
  • Sanitary Clip

If your cat has been seen at another clinic or emergency hospital, please bring the records from that visit OR provide us with contact information so we can request the requests.

If you would like to complete a “Lodging Form” before arrival, it is available on our website.
Prices for lodging are:
  • Day Board $25.00
  • Routine (no meds) $30.00 per night
  • Medication Once daily $34.00 per night
  • Medication twice daily $36.00 per night
  • Medication three times daily $38.00 per night
